About 'united healthcare community plan'|...history of the Church in the United States: its first...the face of the Catholic community in your country has..., educational, healthcare and social institutions...








The               health               care               reform               signed               into               legislation               by               the               Obama               Administration               took               effect               on               September               23rd,               2010.

Children               in               Texas               will               be               significantly               impacted               by               this               change               as               many               of               the               top               insurance               carriers               in               Texas               will               be               doing               away               with               child-only               policies               in               the               wake               of               this               reform.

What               does               Health               Reform               State?
               The               Health               Reform               legislation               will               now               require               all               insurance               providers               to               carry               children               with               pre-existing               conditions.

In               the               past,               many               children               were               denied               coverage               if               they               were               found               to               have               a               pre-existing               condition,               but               thanks               to               the               new               law,               insurance               providers               cannot               deny               them.

However,               these               companies               have               decided               that               in               order               to               prevent               themselves               from               having               to               carry               children               with               pre-existing               conditions,               it               would               be               better               to               do               away               with               all               child-only               policies.

"Child               Only"               policies               are               policies               that               just               cover               children               under               the               age               of               19.
               Who               is               Doing               Away               with               Insurance               for               Children?
               In               Texas,               the               biggest               insurers               are               Blue               Cross               Blue               Shield,               United               Healthcare,               Aetna,               and               Cigna.

Blue               Cross               dominates               44               percent               of               the               Texas               market               and               United               Healthcare               dominates               a               quarter.

United               Healthcare,               Aetna,               and               Cigna               have               all               stated               that               they               will               be               doing               away               with               child-only               policies,               which               means               that               they               will               keep               the               current               policies               until               they               are               up               for               renewal,               but               will               not               renew.

Their               argument               for               this               change               is               that               they               are               trying               to               "keep               family               coverage               affordable               for               as               many               people               as               possible               and               avoid               significant               price               hikes"               and               that               "under               the               new               law,               people               suddenly               seeking               coverage               would               predominately               be               those               who               need               to               consume               health               care               services               immediately               for               known,               high               cost               conditions."               There               are               currently               263,800               Texas               children               under               the               age               of               19               covered               by               these               policies.
               What               is               the               Impact?
               There               is               no               evidence               to               support               that               these               large               insurance               companies               will               suffer               as               a               result               of               this               element               of               health               care               reform.

In               fact,               according               to               the               September               13th               edition               of               the               Houston               Chronicle,               Texas               Representative               Garnet               Colemen               of               Houston               was               quoted               as               saying               that               the               actual               number               of               children               who               have               pre-existing               conditions               and               are               seeking               coverage               is               small.

According               to               the               Kaiser               Foundation,               43.4%               of               Texas               children               are               covered               on               their               parents'               employer               insurance,               30.7               percent               of               children               are               covered               through               Medicaid,               5.8               percent               are               covered               through               individual               or               other               public               plans,               and               20.1percent               are               uninsured.

It               seems               that               these               companies               are               worried               about               the               number               of               children               with               pre-existing               conditions               who               fall               into               that               20.1percent               of               uninsured               Texans.
               The               slogan               for               United               Healthcare               is               "Helping               people               live               healthier               lives"               and               Aetna's               website               states               that               they               are               "committed               to               compliance               with               the               new               legislation,               and               we               are               fulfilling               our               obligation               to               implement               new               benefit               and               health               plan               requirements."               How               exactly               is               United               Healthcare               helping               people               live               healthier               lives               when               they               are               dropping               children               from               policies?

How               is               Aetna               fulfilling               their               obligation               by               coming               up               with               a               new               way               of               denying               children               in               need               of               health               coverage?

These               insurance               giants               have               decided               that               their               bottom               line               is               more               important               than               the               health               of               their               subscribers               and               their               subscribers'               children.

They               claim               that               they               are               trying               to               prevent               a               spike               in               premiums               by               not               allowing               certain               children               coverage,               but               is               that               really               the               case?

According               to               Pauline               Rosenau,               who               is               a               professor               of               Management,               Policy,               and               Community               Health               at               the               University               of               Texas               School               of               Public               Health,               the               "middle               class               will               feel               the               greatest               impact               from               the               change               and               likely               have               to               forgo               coverage               of               their               children               if               their               employer-based               family               plans               cut               coverage               of               dependents."
               If               children               cannot               receive               health               insurance,               parents               will               be               more               reliant               on               emergency               care               and               public               funding               to               take               care               of               their               children,               which               will               pass               along               the               burden               to               the               rest               of               Texas               citizens.

We               need               to               allow               all               people               access               to               health               insurance,               so               we               don't               see               price               increases               in               emergency               room               visits               and               our               own               premiums.

These               insurance               giants               have               always               care               more               about               their               bottom               line               than               their               customers,               and               I               we               need               to               continue               to               support               legislation               to               prevent               them               from               making               our               healthcare               decisions               for               us.

    How               does               technology               affect               healthcare?

    Webster               dictionary               defines               technology               as               the               practical               application               of               knowledge               especially               in               a               particular               area.

    The               motivating               factor               underlying               all               technological               activities               is               the               desire               to               fulfill               a               need.

    Health-care               technologies               have               been               employed               to               extend               human               life,               to               reduce               pain,               disease               risk,               and               to               improve               patient's               quality               of               life.
                   The               following               technologies               are               considered               to               be               health               technology
                   Drugs:               e.g.,               antibiotics,               ACE               inhibitors,               aspirin,               beta-blockers,               antibiotics,                              Biologics:               gene               therapies,               antibodies                              Devices:               e.g.,               pacemakers,               CT               scanners                              Diagnostic               tests:               e.g.

    DNA               fecal               test               for               colorectal               cancer,               genome               sequencing                              Medical               and               surgical               procedures:               e.g.,               psychotherapy,               coronary               angiography,               gastric               banding               operation               for               weight               loss                              Support               systems:               e.g.,               electronic               patient               record               systems,               telemedicine               systems,                              Organizational               and               managerial               systems:               e.g.,               cholesterol               management               program,               billing               and               accounting.

    These               technologies               can               be               used               to               prevent               diseases               (e.g.,               immunization,               chemoprevention               against               cancer,               nutrigenetics),               to               screen               diseases               in               asymptomatic               people               (e.g.,               mammography,               colonoscopy,               cholesterol               testing),               to               diagnose               (e.g.,               electrocardiogram,               x-ray               for               broken               bones),               to               treat               diseases               (e.g.,               gene               therapy,               biologics               for               adjuvant               chemotherapy),               to               rehabilitate               (e.g.,               physical               therapy)               and               to               assist,               to               reduce               errors               (electronic               medical               records,               computerized               order               physician               entry               )

    How               to               evaluate               a               health               technology?

    We               have               to               make               decisions               everyday               about               how               to               spend               our               money,               e.g.

    how               much               we               are               going               to               spend               on               shoes,               on               clothes,               on               food,               how               much               we               are               going               to               give               away               to               charity.

    The               same               thing               happens               in               health               care.

    The               policy               makers               need               to               make               decisions               regarding               which               health               care               technologies               to               fund,               drawing               from               collective               resources.
                   In               the               U.S.,               the               cost               of               health               technology               has               skyrocketed               in               the               past               few               decades.

    Several               novel               drugs               and               procedures               cost               in               the               range               of               hundreds               of               thousand               US               dollars               routinely               prescribed               to               patients.

    Obviously,               no               patients               or               doctor               refuse               access               to               state-of-the-art               technology,               and               although               these               technologies               may               improve/prolong               survival               or               improve               quality               of               life               for               patients,               they               put               a               lot               of               stress               on               the               system               and               reduce               resources               that               can               be               spent               for               other               causes,               e.g.

    preventive               medicine.

    Given               the               limited               resource,               it               is               important               to               give               careful               considerations               to               the               cost-effectiveness               of               health               technology.

    The               growth               of               health               technology               assessment               (HTA)               in               the               U.S.

    reflects               this               demand.
                   Health               technology               assessment               is               a               process               designed               to               "provide               decision               makers               with               information               about               the               possible               impacts               and               consequences               of               a               new               technology               or               a               significant               change               in               an               old               technology.

    It               is               concerned               with               both               direct               and               indirect               or               secondary               consequences,               both               benefits               and               disbenefits,               and               with               mapping               the               uncertainties               involved               in               any               government               or               private               use               or               transfer               of               a               technology.
                   HTA               was               first               developed               in               U.S.,               but               has               gained               popularity               worldwide.

    In               the               United               Kingdom,               National               Institute               for               Health               and               Clinical               Excellence               (NICE)               decides               whether               a               new               technology               will               be               used               in               the               National               Health               Service               (NHS)               based               on               its               cost-effectiveness               analysis.

    The               stake-holders               of               HTAs               include
                   Regulatory               agencies,               e.g.

    about               whether               a               drug,               device               or               technology               is               permitted               for               commercial               use               Health               care               payers,               providers,               and               employers,               e.g.

    about               whether               a               technology               should               be               reimbursed.

    Hospitals,               health               care               networks               Standards-setting               organizations               for               health               technology               and               health               care               Government               health               department               officials               about               public               health               programs               Lawmakers               and               other               political               leaders               Health               care               product               companies               Clinicians               and               patients               
    Often,               several               fields               of               expertise               are               required               to               perform               HTA.

    So               a               HTA               may               involve               a               wide               range               of               experts,               from               physicians,               nurses,               dentists,               managers,               technicians,               biomedical               and               clinical               engineers,               pharmacologists,               patients               and               patient               affairs               representatives,               epidemiologists,               biostatisticians,               economists,               lawyers,               to               social               scientists,               computer               programmers.

    About 'united healthcare providers'|United Healthcare Settlements Part II - A Nationwide Healthcare Database








                   My               parents               are               approaching               retirement               and               planning               to               move               to               a               more               rural               area               of               the               United               States.

    With               a               report               by               the               American               Association               of               Medical               Colleges,               it               is               estimated               that               we               will               need               more               than               20,000               new               primary               care               physicians,               in               rural               areas,               to               meet               the               demand               of               retirees.

    Based               on               this               information,               I               have               tried               to               address               these               concerns               with               my               parent's               health               care               access               and               feel               you               may               want               to               do               the               same.
                   While               the               cost               of               living               in               rural               America               is               typically               cheaper,               if               you               have               a               pre-existing               health               condition               there               could               be               challenges               to               acquiring               quality               care.

    Lack               of               doctors               is               the               primary               focus               of               the               report               by               the               AAMC,               in               which               they               acknowledge               that               physicians               in               rural               areas               are               typically               not               working               in               specialty               as               a               wider,               and               more               available,               patient               base               is               found               in               traditional               family               practice.

    For               my               parents,               if               there               are               any               specialty               healthcare               services               they               need,               while               living               in               a               rural               area,               it               may               be               more               challenging               to               find               the               care               they               need,               especially               in               emergency               situations.
                   In               addition               to               lack               of               specialized               services               in               rural               areas,               another               cause               for               concern               is               the               lack               of               available               health               services               accepting               Medicare               as               a               form               of               payment.

    Because               the               federal               Medicare               program               typically               pays               for               rural               healthcare               expenses               at               a               rate               less               than               that               of               the               urban               healthcare               system,               healthcare               services               are               limited.

    While               there               may               be               physicians               and               health               providers               in               a               town               that               my               parents               decide               to               live               in,               the               fact               remains               that               some               of               the               health               services               may               be               prohibitive               for               them               based               upon               the               lack               of               Medicare               acceptance.
                   To               mitigate               these               financial               and               healthcare               access               risks               -               and               if               my               parents               continue               to               insist               that               retiring               to               a               rural               area               is               more               ideal               -               I               will               try               to               persuade               a               specific               move               that               will               provide               more               access               to               services.

    If               you               are               facing               the               same               dilemma,               try               to               find               areas               in               which               nurse               practitioners               are               abundant               and               work               to               encourage               a               move               to               a               state               where               nurse               practitioners               can               practice               care               without               the               direct               supervision               of               a               doctor.

    In               doing               so,               we               may               have               a               greater               opportunity               to               find               our               parents               the               healthcare               they               need               while               also               supporting               their               move               to               rural               living.

    Nurse               practitioners               may               be               in               more               abundance               and               pick               up               the               significant               shortage               of               physicians               we               need               in               these               areas.
                   Living               in               tiny               houses,               and               transitioning               to               rural               living               has               become               a               growing               trend               in               the               United               States.

    If               your               parents               are               like               mine,               and               if               you               are               concerned               about               their               health               care               access,               consider               these               disadvantages               and               work               to               mitigate               the               complication               by               finding               the               best               rural               area               with               doctors               and               nurse               practitioners,               in               specialty,               ready               to               take               Medicare               as               payment.
